Care Management

We provide coordination of care and resources for our patients during a hospital stay at New London Hospital (NLH) and in our clinics. We work to ensure you receive the support that you need to achieve your healthcare goals.

Our services

We have a variety of roles and services to help support your care.

  • Activities coordinators help patients receive individualized activities during their hospital stay. Patients receiving activities are undergoing skilled nursing or rehabilitation care.
  • Interpreting services are available to ensure you can participate in your care. Our services support:
    • Interpreting and translation to your preferred language
    • Communication for those using various modalities
  • Registered nurses work with providers, nurses, and therapists to support you during your hospital stay. From admission through discharge, registered nurses coordinate care and services after you leave the hospital.
  • Social workers help patients and families cope with the impact of illness or injury. They can assist to refer you to financial or community resources to help you be successful after discharge from the hospital. Social work can provide support in the inpatient setting as well as in the clinics.
  • Transitional care coordinators are nurses who support care for our primary care patients during a hospital stay. They ease communication between your primary care provider at NLH and your inpatient team. Transitional care coordinators ensure you have the information and support that you need after you leave the hospital. They follow up with you within 2 business days after your discharge

Our office at NLH is open from 8:00 am to 5:00 pm, Monday through Friday.
