In an effort to personalize and demystify the healthcare experience for patients and families, New London Hospital has created a Patient and Family-Centered Care Task Force, chaired by Trish Sweezey, RN, Senior Director of Nursing and Susan Bryant, Senior Director of Community Relations. The focal points of the Task Force are dignity and respect, information sharing, involvement and collaboration.
"We are working towards the formal adoption of policies that places the patient and family at the center of all we do," Sweezey explained. "This involves fully integrating the family’s preferences and concerns into our plan of care, encouraging the family to become involved, and working together with healthcare staff to develop and evaluate policies and procedures."
Specific components of Patient and Family-Centered Care include human interactions, the nurturing aspects of food, spirituality, the human touch, and healthy communities. As the Task Force completes almost a year of work, accomplishments include a new vision and mission statement for the hospital, and the implementation of a Patient & Family Advisor program.
